New Employee Checklist

Below please find several documents that we ask you to fill out and sign as we begin to set up your employee file.

Tax form prepared by an employee indicating the employee’s exemptions and Social Security number, and enabling the employer to determine the amount of taxes to be withheld for the employee. Please complete both Federal and VT forms.

The form that is required by the Department of Homeland Security – U.S. Citizenship and Immigration Services to document eligibility for employment in the United States. All new employees, including U.S. citizens, must supply documents to identify themselves and to establish their eligibility for U.S. employment. For questions please click the I-9 Info Page.

For your convenience, we provide direct deposit. Your paycheck will be automatically deposited into the account(s) you indicate.

This form should list anyone who you would like us to contact, and with whom you will allow us to share information about your situation in case of any emergency.

These forms are required to be signed indicating you understand to whom and what you can disclose about WBC products.  Please make sure you complete for the company you will be working for.
